Adult Indemnity form – KICK for Young Lives vs Cancer 2023

• J D Wetherspoon Plc / Young Lives vs Cancer – KICK tournament 2023 – Sunday 17 September 2023.
• Please note that the event that you have registered to take part in requires a certain level of fitness. When registering to take part in this event you do so at your own risk, and therefore you should be fit to participate. If you are in any doubt please seek further information about what the event entails; seek medical advice if appropriate and inform event organisers of any relevant medical conditions you have. You should advise the event organisers immediately of any medical condition arising after registration that might affect your participation and you should withdraw from the event if you have any doubts about your fitness to participate.
• Goals Soccer Centre plc, Powerleague and Astropark Coolock are running this event independently of J D Wetherspoon. J D Wetherspoon has no responsibility for the equipment supplied, the instruction and supervision given, the safety procedures or the suitability of the venue in relation to this event. J D Wetherspoon and its employees, agents and volunteers are under no liability whatsoever to participants or spectators in respect of any loss or injury which may be sustained in the course of this event.
• J D Wetherspoon accepts no liability for injury to the participant however caused although J D Wetherspoon will take all such steps as are reasonable to ensure the participant’s safety while under J D Wetherspoon immediate control. J D Wetherspoon accepts no responsibility for any loss or damage to the personal belongings of any participant. J D Wetherspoon accepts no responsibility of participants travel to and from the event or of overnight accommodation they may book.
• I would like to take part in the football tournament at Goals Soccer Centre plc, Powerleague or Astropark venue. I realise that Goals Soccer Centre plc, Powerleague and Astropark are running the football tournament independently of J D Wetherspoon as an independent contractor and not the agent or servant of J D Wetherspoon. I understand that J D Wetherspoon has no responsibility for the equipment supplied, the personnel used by Goals Soccer Centre plc, Powerleague or Astropark the instruction and supervision given, the safety procedures, ensuring the suitability of the Goals Soccer Centre plc, Powerleague or Astropark venue or otherwise in relation to the football tournament. I therefore acknowledge that J D Wetherspoon bear no responsibility for my safety during the period that I am under the supervision of Goals Soccer Centre plc, Powerleague or Astropark.
• I also acknowledge that J D Wetherspoon accepts no responsibility of participants travel to and from the event or of overnight accommodation they may book. I understand that the event may be cancelled or that I may be stopped from participating in the football tournament on the grounds of safety.
• I consent to a £10 donation to be made from my payslip for this event
• I hereby confirm that I accept the above terms for participating in the J D Wetherspoon 2023 KICK for Young Lives vs Cancer event.
